As a nurse-managed and owned company, we are focused on the complete care of our clients and families. If navigating the complex healthcare system seems daunting, we are here to help. All of our familes receive healthcare navigation assistance at no extra charge.
What's Included
Teaching self-management strategies (e.g., medication adherence, symptom monitoring)
Connecting patients with counseling, support groups, or palliative care as need
Helping patients articulate concerns and preferences to their healthcare team
Identifying and advising on challenges such as transportation and insurance navigation
Supporting adherence to treatment plans and follow-up care
Helping with transitions (e.g., hospital to home, or active treatment to survivorship care)
Ensuring care aligns with the patient’s goals and values.
Linking to community resources and social services
Serving as a consistent point of contact throughout the care journey
Connecting patients with nutrition, or support groups
