This program ensures a safe and successful transition from rehab to home.​
Our experienced caregiver will be with you for 4 hours to assist with the following:
Ride Home from Rehab
We will provide safe and supportive transportation from the facility to home. Once home, we will assist with a safe transition from the car to your favorite spot in the house!
Picking up Prescriptions
What's Included
Connecting patients with counseling, support groups, or palliative care as need
We will pick up your new prescriptions and make sure they are ready for you when you need them.
Grocery Shopping
We will make sure your refrigerator and pantry are stocked with all your favorite foods.
Meal Preparation
We will prepare a hot meal for your first meal home.
Personal Care
We are happy to assist with a shower and/or any personal care to help you feel settled in at home.
Next Day Check-in Call
We will call you the next day to make sure you have everything you need for continued success!
For $195, you will have safe transportation, your medications, groceries, a hot meal and PEACE OF MIND!
